ABSTRACT

OBJETIVO: Determinar si la ansiedad preoperatoria supone un factor de riesgo independiente para la percepción de dolor severo durante la realización de una histeroscopia quirúrgica ambulatoria. MATERIAL Y MÉTODOS: Estudio de cohortes en el que incluimos179 pacientes. Se ha aplicado el formulario STAI-S para valorar la ansiedad preoperatoria de las pacientes y distribuirlas en dos cohortes: Pacientes con ansiedad (STAI-S >50) y pacientes sin ansiedad (STAI-S7. Se ha calculado la asociación y el riesgo entre ambas variables mediante el Test X2 y el Riesgo Relativo (RR). Se ha utilizado el test de correlación de Pearson para valorar la correlación entre ambas variables. Se ha considerado estadísticamente significativo un valor de p<0.05. RESULTADOS: Las pacientes que percibieron dolor severo durante la entrada a la cavidad uterina (78,3% vs 29,5%; p<0,001) y durante la realización del proceso quirúrgico (78,8% vs 26,2%; p<0,001) fueron en su mayoría pacientes con ansiedad preoperatoria. El RR de las pacientes con ansiedad para percibir dolor severo es de 6,46 (IC 95%; 2,52 -16,60) durante la entrada y de 6,61 (IC 95%; 3,04 -14,38) durante la resección. Existe una correlación moderada y positiva entre las puntuaciones obtenida en la escala STAI-S y la puntuación EVA, tanto durante la entrada a la cavidad (r = 0,629; p = 0,042) como durante el proceso quirúrgico (r = 0'661; p =0'021). CONCLUSIONES: Las pacientes con ansiedad preoperatoria tienen más riesgo de percibir dolor severo durante la entrada en la cavidad uterina y durante la realización de la intervención histeroscópica.


OBJECTIVE: Determine if preprocedural anxiety is an independent risk factor for the perception of severe pain during an outpatient surgical hysteroscopy. MATERIAL AND METHODS: There were 179 patients included in this cohort study. The STAI-S questionnaire has been applied to assess the preoperative anxiety of the patients and distribute them into two cohorts: Patients with anxiety (STAI-S>50) and patients without anxiety (STAI-S 7. The association and the risk between both variables have been calculated using X2 test and relative risk (RR). Pearson's correlation test was used to assess the correlation between both variables. A p value < 0,05 has been considered statistically significant. RESULTS: Patients who perceived severe pain during access to the uterine cavity (78,3% vs 29,5%; p<0,001) and during the surgical procedure (78,8% vs 26,2%; p<0,001) were mostly patients with preoperative anxiety. The RR of patients with anxiety to perceive severe pain is 6.46 (95% CI; 2,52-16,60) during access to the uterine cavity and 6,61 (95% CI; 3,04-14,38) during resection. We have found a moderate and positive correlation between the scores obtained on the STAI-S questionnaire and VAS score during access to the uterine cavity (r=0,629; p=0,042) and during the surgical process (r=0,661; p=0,021) CONCLUSIONS: Patients with preprocedural anxiety are more at risk of perceiving severe pain during access into the uterine cavity and during the hysteroscopic intervention.

ABSTRACT Objective: To identify the predominant chronotype in nursing workers who work in surgical clinics of university hospitals and to verify the association with work shift. Method: Cross-sectional study, performed in surgical clinics of university hospitals in the Southern region of Brazil. The sample of 270 nursing workers answered questions of socio-occupational characterization, of health and the Morningness-Eveningness Questionnaire of Horne and Östberg. We performed a descriptive and bivariate analysis with the help of the SPSS software and confidence interval of 95%. Results: The indifferent chronotype predominated (45.2%). There were significant differences between occupational categories and variables "age" (p<0.001), "use of medication" (p=0.035) and "choice of work shift" (p=0.001). There was an association between the chronotype and the variables "work leave due to illness" (p=0.021), "children" (p=0.025), "use of medication" (p=0.018) and "work shift" (p=0.001). Conclusion: The chronotype remained indifferent, and the results confirmed association between chronotype and work shift.


RESUMEN Objetivo: Identificar el cronotipo predominante en trabajadores de enfermería que actúan en clínicas quirúrgicas de hospitales universitarios y verificar asociación con el turno de trabajo. Método: Estudio transversal realizado en clínicas quirúrgicas de hospitales universitarios de la región sur de Brasil. La muestra de 270 trabajadores de enfermería respondió cuestiones de caracterización sociolaboral, de salud y el Cuestionario de Matutinidad-Vespertinidad de Horne y Östberg. Se realizó un análisis descriptivo y bivariado con ayuda del software SPSS y un intervalo de confianza del 95%. Resultados: Predominó el cronotipo intermedio (45,2%). Se observó una diferencia significativa entre las categorías profesionales y las variables "edad" (p <0,001), "uso de medicación" (p = 0,035) y "opción por el turno de trabajo" (p = 0,001). Se identificó asociación entre el cronotipo y las variables "alejado del trabajo por motivo de enfermedad" (p = 0,021), "hijo (s)" (p = 0,025), "uso de medicación" (p = 0,018) y "turno de trabajo" (p = 0,001). Conclusión: Predominó el cronotipo intermedio, y los resultados confirmaron la asociación entre cronotipo y turno de trabajo.

Método: Estudo transversal, realizado em clínicas cirúrgicas de hospitais universitários da região Sul do Brasil. A amostra de 270 trabalhadores de enfermagem respondeu questões de caracterização sociolaboral, de saúde e o Questionário de Matutinidade-Vespertinidade de Horne e Östberg. ABSTRACT

ABSTRACT Objective: To identify pediatric caregivers' reactions in outpatient surgery settings. Methods: A quantitative descriptive/exploratory survey-based study involving application of a semi-structured questionnaire to 62 caregivers in two hospitals. Results: Most caregivers (88.7%) were mothers who submitted to preoperative fasting with their children. Nervousness, anxiety and concern were the most common feelings reported by caregivers on the day of the surgery. Conclusion: Medical instructions regarding preoperative procedures had significant positive impacts on patient care, and on patient and caregiver stress levels.


RESUMO Objetivo: Identificar as reações dos acompanhantes de crianças submetidas à cirurgia ambulatorial. Métodos: Estudo survey descritivo/ exploratório, de caráter quantitativo, realizado em dois hospitais com 62 acompanhantes que responderam um questionário semiestruturado. Resultados: Constatou-se que a maioria dos acompanhantes era formada por mães (88,7%) e permaneceu em jejum junto das crianças. Todos os acompanhantes referiram sentimentos identificados como nervosismo, ansiedade e preocupação. Conclusão: A orientação médica aos procedimentos pré-operatórios foi de grande importância, não somente para o cuidado da criança, mas também do acompanhante, visando diminuir ao máximo o estresse vivenciado por eles.

ABSTRACT

OBJETIVO: Verificar a influência das atividades lúdicas realizadas durante o pré-operatório sobre a ansiedade de crianças participantes do projeto de recreação terapêutica desenvolvido no Centro Cirúrgico Ambulatorial (CCA) do Hospital de Clínicas de Porto Alegre, Porto Alegre (RS). MÉTODOS: Para a realização do estudo, foi aplicada a Escala de Ansiedade Pré-operatória de Yale modificada (modified Yale Preoperative Anxiety Scale, mYPAS) em 50 crianças de 5 a 12 anos submetidas a procedimentos médicos no CCA. As crianças foram divididas em dois grupos: grupo recreação (participou de intervenções lúdicas na sala de recreação) e grupo controle (não participou de intervenções lúdicas). A ansiedade foi mensurada através da escala mYPAS em dois momentos: logo que os indivíduos chegaram ao CCA (minuto 0) e 15 minutos após a primeira medida. RESULTADOS: Os resultados mostraram que, logo ao entrar no CCA, os pacientes não apresentaram diferenças significativas com relação à ansiedade, sendo os valores elevados. No entanto, após 15 minutos de espera ou intervenção, as crianças do grupo recreação diminuíram a ansiedade, e as do grupo controle permaneceram ansiosas. CONCLUSÕES: Durante o período pré-operatório, crianças que participam de atividades lúdicas na sala de recreação diminuem a sua ansiedade em comparação àquelas que somente ficam na sala de espera pelo menos durante 15 minutos.


OBJECTIVE: To verify the influence of playful activities during the preoperative period on the anxiety of children participating in the therapeutic recreation project conducted at the outpatient surgical center of Hospital de Clínicas de Porto Alegre (HCPA), Porto Alegre, Brazil. METHODS: The modified Yale Preoperative Anxiety Scale (mYPAS) was administered to 50 children between 5 and 12 years of age undergoing medical procedures at the outpatient surgical center. The children were divided in two groups: recreation group (individuals who participated in playful interventions in the recreation room) and control group (individuals who did not participate in playful interventions). Anxiety was measured using the mYPAS at two different moments: soon after the individuals arrived at the outpatient surgical center (minute 0) and 15 minutes after the first measurement. RESULTS: The results showed that soon after arriving at the outpatient surgical center, the patients did not have significant differences regarding anxiety, showing high values. However, after 15 minutes waiting or undergoing intervention, the children in the recreation group had reduced anxiety levels and those in the control group remained anxious. CONCLUSIONS: During the preoperative period, children who participate in playful activities in the recreation room have their anxiety reduced in comparison with those that only stay in the preoperative holding area for at least 15 minutes.

ABSTRACT

Se exponen los resultados alcanzados en la cirugía mayor ambulatoria del Servicio de Cirugía Maxilofacial del Hospital Universitario "Dr Miguel Enríquez" en el año 2003, con el objetivo de evaluar el comportamiento de esta actividad y su repercusión institucional y económica. Se realizó un análisis prospectivo de observación considerando las variables edad, sexo, tipo de anestesia, intervención quirúrgica realizada y complicaciones, las cuales fueron organizadas, analizadas y tabuladas por el método simple de los palotes. El 61 por ciento de los pacientes fue operado por este proceder, todos con anestesia local. Los indicadores hospitalarios mejoraron al reportarse por esta vía el 58,32 por ciento del total de intervenciones quirúrgicas. El sexo femenino (63,38 por ciento) y los grupos de edades de 15-24 y 35-44 años, fueron predominantes en nuestra serie. Las extracciones múltiples con alveoloplastias y la exéresis de los dientes retenidos, fueron las intervenciones quirúrgicas de mayor frecuencia, con el 48,02 y 38,60 por ciento, respectivamente. No hubo complicaciones significativas. Se comparan los resultados con el de otros autores, que son muy similares en el orden cuantitativo. Se evalúa el impacto económico-social por el importante ahorro de recursos que representa este método, fundamentalmente al no haber gastos por hospitalización, y en la familia, que solo deja de trabajar el día de la operación, además de preferir este método por tener mayor acercamiento con el paciente(AU)


The results attained in the ambulatory major surgery of the Maxillofacial Surgery Service of "Dr. Miguel Enríquez" Teaching Hospital in 2003 are exposed aimed at evaluating the behavior of this activity and its institutional and economic impact. The method consisted in an observational prospective analysis, considering variables such as age, sex, type of anesthesia, surgery performed and complications, which were organized, analyzed and tabulated by the downstroke method. It was observed that 61 percent underwent this surgical procedure with local anesthesia. The hospital indicators improved on being reported by this way 58.32 percent of the total of operations. Females (63.38 percent) and the age groups 15-24 and 35-44 predominated in our series. Multiple extractions with alveoloplasties and the exeresis of the retained tooth were the most frequent operations, with 48.02 and 38.60 percent, respectively. There were no significant complications. The results are compared with those obtained by other authors that were very similar in the quantitative sense. The economic-social impact is evaluated due to the significant saving of resources this method represents., since there are no hospitalization costs and the relatives only have to be absent from work the day of the operation. They also prefer this method because they are closer to the patient(AU)

ABSTRACT

Os objetivos deste estudo foram identificar o estado de ansiedade e os mecanismos de coping utilizados por pacientes cirúrgicos ambulatoriais no período pré-operatório, suas manifestações clínicas e respectivas correlações. Os resultados obtidos identificam que a amostra de 40 pacientes apresentou baixa ansiedade e o mecanismo de coping mais utilizado foi o suporte social, porém, não obtivemos relação estatisticamente significante entre estado de ansiedade e mecanismos de coping e alterações clínicas.


The goals of this study were to identify the anxiety state in surgical ambulatory patients in the pre-operative period, the coping mechanisms employed, their clinical manifestation, and to verify the influence of state anxiety during the pre-operative period. The results showed that patients exhibited little anxiety and there was no significantly statistical correlation between pre-operative anxiety state, coping mechanisms and clinical changes.


Este estudio tuvo como objetivos identificar el estado de ansiedad y los mecanismos de "coping" utilizados por pacientes quirúrgicos de consulta externa en el período pre-operatorio, sus manifestaciones clínicas y respectivas correlaciones. Los resultados obtenidos identifican que la muestra de 40 pacientes presentó baja ansiedad y el mecanismo de coping más utilizado fue el soporte social, sin embargo, no se obtuvo relación estadísticamente significativa entre el estado de ansiedad y mecanismos de "coping" y alteraciones clínicas.

ABSTRACT

Existe la necesidad de utilizar una medicación preanestésica, útil para los niños que seran sometidos a cirugía electiva ambulatoria, está dene ser fácilmente aceptada y de preferencia por vía oral. Se evaluó la dosis de midazolam 0.75 mg/km por vía oral en forma de golosina con sabor a chocolate, en un grupo de 20 niños de 1 a 10 años de edad, comparado con otro grupo similar el cual no recibió chocolate ni medicamento. Se observó el grado de sedación a los 15 y 30 minutos, la calidad de la separación familiar y la facilidad para la aplicación de la venopunción. La aceptación de la estancia hospitalaria y el grado de amnesia en niños mayores de 5 años de edad. El midazolam 0.75 mg/Kg produce sedación a los 20 a 30 minutos después de su administración y no prolonga el tiempo de recuperación, por lo tanto se concluye que el midazolam 0.75 mg/Kg por vía oral es efectivo para el medicación preanestésica en pacientes pediátricos programados para cirugía ambulatoria.
